Good Faith Estimates and No Surprises Act | CMS
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. The estimate is based on information known at the time the estimate was created. The GFE does not include any unknown or unsuspected costs that may arise during treatment. You could be charged more if complications or special circumstances occur and will be provided a new "Good Faith Estimate" should this occur.
You’re getting this notice because this provider or facility isn’t in your health plan’s network and is considered out-of-network. This means the provider or facility doesn’t have an agreement with your plan to provide services. Getting care from this provider or facility might cost you more than utilizing your in-network insurance benefits.

Therapy
Therapy services are as follows:
Intake: $130
Individual 60 minute: $130
Couples 50 minute: $150
Individual 45 minute: $110
Insurances accepted: Triwest, Tricare, Cigna, Blue Shield, Aetna, Magellan, Cencal, Holman Group

Assessment/ Independent Educational Evaluation
Evaluation fees- $130 per hour. Hours include initial consultation, interviews, record review, direct testing, scoring, report writing, recommendations, and feedback session to review the results. I am very thoughtful when analyzing data, so know that this takes time. Therefore, it is crucial I obtain a good idea of concerns during the initial consultation so I can develop a selective, focused-based testing plan that thoroughly addresses the stated referral problem. After the initial meeting I provide an estimate of the maximum number of hours the evaluation will take.
Consultation and meeting after the review/write up will also be part of the estimate.

Autism/ADHD assessment
This assessment is focused on gathering information through interviews, rating scales, and direct testing to see if you are your child is impacted by Autism and/or ADHD. The assessment typically will take 2 assessment appointments to complete interviews, intelligence testing, and gather information about sensory strengths and needs. Assessment appointments last from 2 to 3 hours. For Adults participating in testing, bringing a friend or family member who knows you well to the initial appointment is helpful

Learning Disorder
Available in-person. This comprehensive assessment will identify if you or your child are impacted by a learning disorder in reading, writing, and/or math. The assessment typically will take three assessment appointments to complete interviews, intelligence testing, academic testing, and cognitive processing assessments. Assessment appointments last from 2 to 3 hours.
